I think the lighting looks great. Just an extra step, I'd be tempted to go in and use the smudge tool a bit to blur out some of the hairs in some areas (particularly where they start and stop.) otherwise, I can't say much else because mine ain't any better... (If I have time tonight I'll post it up too.)

On your bald woman assignment, there are two things that I feel will improve the way it looks now.
One will be to have some variation of tones. Right now all the bright areas are very similar in value and the same thing goes for the dark ares. Some values in between the two will help vary things up.
The other thing will be to add some individual strands of hair here and there. Just very sparingly, in the same general direction of the locks of hair that you've already painted.
